Tween Book Bags Registration

This registration is for Tween Book Bag Subscriptions for Summer 2024. 

Tween Book Bags are for ages 8 - 12.

Each bag will contain

• A Library Book
• Review Card
• Bookmark
• Small Prize

When you’re done reading, just return the Library book, all other items in the bag are yours to keep!

​Rules:

• You must have a Uintah County Library card.

• You may not have fines greater than $9.99 on the library card you provide.

• All forms that are emailed to you must be filled out by the end of registration or your book bag may be canceled. Please make sure to check your spam inbox. 

• You or a parent/guardian must be able to pick it up in person.

• If you do not pick up your bag after 2 weeks from the pick-up date your items will be put back and your registration canceled. 

This registration will last for June, July, and August.

 Reminder: If you fail to fill out the questions in the form that's emailed to you by the final registration date, your book bag will be canceled. If you cannot access the form, please e-mail psroka@uintah.utah.gov or stop by the Library and ask for Pamela.
